Client centred care to help you achieve your goals.

Rehabilitation after a stroke can be a long and challenging journey, but it is also a journey of hope and progress. We have the privilege of working with a lady who had experienced a stroke affecting her right side. After being discharged from the hospital, she was unable to return home due to her difficulties following the stroke and was placed into a care home for continued rehab. However, she was not receiving as much input from the Therapy teams in the nursing home as she would have liked, and her mood was being affected. Her family contacted us to see how we could help.

At Bristol Home Therapists, we were able to work alongside the NHS Therapists to work on the shared goals set by her. We initially saw her twice a week in the care home setting, where she was bed-bound and unable to move. Rehabilitation initially focused on building confidence and the ability to sit out in a chair, which allowed her to eat her meals, engage with family and friends, and helped with her speech. Therapy sessions consisted of sitting and standing practice, encouraging hand and arm movements through functional tasks and games!

With a lot of hard work from her and practice, this allowed her to go home with a care package with the ability to stand using a piece of equipment to help with transfers. Once home, rehabilitation continued, and she gained both activity, strength, and improvement in balance, which now allows her to walk with a 4-wheeled walker independently around the house and outside to the car. Within therapy sessions, we continue to focus on building confidence, strength, and balance when mobilising with a walking stick through exercises, games, and daily activities. Her long-term aim is to walk with her walking stick within the house independently.

It is important to note that rehabilitation after a stroke is a highly individualised process that depends on the symptoms and their severity 1It often starts in the hospital and continues at home or at a local clinic in your community 1A team of different specialists may help with your rehabilitation, including physiotherapists, psychologists, occupational therapists, speech and language therapists, dietitians, and specialist nurses and doctors 1You’ll be encouraged to actively participate in the rehabilitation process and work with your care team to set goals you want to achieve during your recovery 1.
