Your appointment(s)

All assessments will take place at your home and clinicians will be wearing appropriate personal protective equipment (PPE).

Prior to any assessment taking place a triage telephone call will be completed by a therapist to understand your needs and which of our services would best support you.

Initial Assessment- Joint Occupational Therapy and Physiotherapy assessment in your own home

1 Hour 45 minutes

Visit include:

• Assessment of function in your own home environment

• Falls assessment, environmental assessment, advice and recommendations

• Identification of equipment, advice and signposting to privately purchase

• Personalised home exercise programme

• Treatment plans and goal setting

• Referrals to other specialists if required

• Discuss of further visits if identified

Follow up home visit - Joint Occupational Therapy and Physiotherapy

45 minutes - 1 Hour

Visit includes:

• Continuation of treatment

• Re-assessment and evaluation of progress and current treatment plan, goals

• Review and progress exercises programme

• Review equipment

• Discuss further visits if identified

If we do not believe that a joint assessment is suitable then an individual Occupational Therapy or Physiotherapy Assessment can be completed.

Individual Occupational Therapy and Physiotherapy sessions

These are available and will be recommended if we don’t feel that a joint session is required

Cancellation

Please note all cancellations need to be made 24 hours in advance otherwise the full cost of the appointment will unfortunately still be charged.
