Our Occupational Therapy and Physiotherapy Services

The Bristol Home Therapists team have a wealth of experience supporting patients with a wide variety of health needs. Bristol Home Therapists can offer assessment and rehabilitation within the comfort of your own home (this can include care home setting either short or permanent stay) for variety of reasons . These may be :

  • Neurological conditions such as Stroke, Multiple Sclerosis, Parkinson’s Disease, Spinal Cord Injury.

  • Orthopaedic Rehabilitation- post knee or hip replacement, fracture.

  • Managing long term conditions

  • Osteoarthritis, Rheumatoid Arthritis

  • COPD

  • Falls prevention

  • Wheelchair and seating

  • Assisting people to return home following a prolonged period in hospital or a respite placement

  • Identification of specialist equipment to aid independence and safety in all aspects of your day

  • Building confidence and independence with outdoor mobility. This would include looking at access in/out of your home and walking aid advice

Joint Therapy assessments

Bristol Home Therapists offer a unique joint assessment which is invaluable for all individuals to receive a full therapeutic assessment , setting a plan to achieve their full potential. The visit would consist of a full mobility, transfers, manual handling and a home environment assessment.

One area of this is the prevention of falls. Working together we would complete an assessment and identify areas of risk. This may be home environment changes or equipment needs, an assessment of your strength and balance and advice on suitable walking aids.

Occupational Therapy assessment

Occupational therapy helps people retain their confidence and independence helping them continue with daily activities independently. The Occupational Therapist will look at your mental and physical health, as well as social and environmental factors.

They will assess whether simple changes in your daily routine could help achieve independence and whether there is a benefit from specialist equipment to aid independence within your home and community.

Physiotherapy assessment

The physiotherapy assessment will look at strength and balance, range of movement, mobility /transfer assessment within the home and community.

An assessment of people with neurological conditions with tonal changes can be completed.

 

Follow up appointments

Bristol Home Therapist service will continue to work with you through follow up occupational therapy and /or physiotherapy appointments if identified at the initial consultation visit to enable you to achieve your identified goals.
